本人，              授權及同意中國教育部留學服務中心可就我本人學歷學位認證的有關事宜向相關機構或組織透露本人的個人資料并對本人取得國（境）外學歷學位證書及高等教育文憑的情況進行核查。同時也授權相關組織或機構可就這些核查內容，向中國教育部留學服務中心透露任何有關的記錄及資料。

Declaration

I，            authorize and consent to the Chinese Service Center for Scholarly Exchange （CSCSE）making any enquiries, which requires releasing my personal information, for purposes relating to qualification evaluation. I also authorize and consent to relevant organizations/schools to release my record or information to the CSCSE as may be required for these enquiries.
